By submitting your application you are agreeing to allow Lender(s) to verify information that you have entered, including, but not limited to, obtaining your credit report and that of any consignors and/or contacting your employer or any consignors to verify employment and income.
You understand and agree that the Lender (as defined in the Promissory Note or communication to you) can furnish information concerning my account to consumer reporting agencies and others who may properly receive that information. Furthermore, you are signifying that you have read this notice and that all of the information submitted on the loan application is true and correct.
There are risks related to any dental/medical procedure which you have discussed with your dentist/practitioner and you acknowledge that neither PatientLink/B3Dental nor any of its employees or representatives have made any representation as to the merits or consequences that may result from the dental treatment, you have not relied upon PatientLink/B3Dental in connection with the treatment and you have relied solely upon your dentist/practitioner with respect to all health-related issues.
Your practitioner and his or her employees do not act as an agent of PatientLink and none of them has any right or power to alter the Loan terms or to make any representation or amendment on behalf of PatientLink.
You authorize PatientLink and/or its financial partners to advise your practitioner whether the Loan has been approved, and you authorize your practitioner to advise PatientLink and/or its financial partners when treatment has been completed and of any other information relevant to the Loan which is not confidential personal medical information.
The rights of PatientLink and/or its financial partners under the Loan will not be affected or limited in any way by any claim that you may have against your practitioner and you understand the Loan is a separate transaction, independent of your dental/medical treatment or other dealings between your practitioner and you.
Dentist/Doctor and other health provider information found on PatientLink/B3Dental is both self-reported, by the physician/dentist and/or physician/dentist office staff, and collected from multiple other data sources that may not be confirmed by the physician/dentist. The information contained on PatientLink/B3Dental web sites is intended for general reference purposes only. PatientLink/B3Dental does not provide advice, recommendations or qualification certification about any particular physician, dentist or provider. You are advised to verify information about a particular physician, dental or medical provider by confirming with the doctor's office, your current physician, dentist, the medical association relative to the doctor's specialty, and your state/provincial medical/dental board. By using this service, the Applicant/Borrower agrees to indemnify and hold harmless PatientLink/B3Dental and its banking partners, for any and all claims whatsoever made against PatientLink/B3Dental by the Applicant which arises or is alleged to arise out of any procedure involving the Provider.
PatientLink/B3Dental may collect personal information from you, from product and service arrangements you have made with our through PatientLink/B3Dental from credit bureaus and other financial institutions and from references you have provided.
PatientLink/B3Dental may collect, use and disclose your personal information for the following purposes: to provide additional financial services, to understand your financial and banking needs, to develop, manage and deliver dental or other health-related products and services, to determine your eligibility for different products and services, to ensure that you receive a high standard of service and to meet regulatory requirements.
PatientLink/B3Dental may give it to credit bureaus and other financial institutions and, with your consent, to other parties. PatientLink/B3Dental may share your personal information with any who works with PatientLink/B3Dental or for PatientLink/ B3Dental, but only as needed for the provision of products and services you request from PatientLink/B3Dental.
PatientLink/B3Dental may also use your personal information to introduce products and services that may be of interest to you through direct marketing (via mail, telephone calls or email only) and share it for marketing purposes with PatientLink/B3Dental business partners and within the B3Dental group of affiliated companies. At any time, you can choose to opt out of receiving PatientLink/B3Dental direct marketing or the sharing of your personal information for marketing purposes.
